COVID-19 Information
Employee Travel Information Form
Employees traveling for personal or approved business reasons:
- YWCA requests your assistance in providing information about your upcoming travels. The association will not prohibit travel to any destination but may require staff members to isolate/quarantine themselves before returning to a YWCA operated facility. This request of information is to ensure YWCA is able to provide a safe and healthy environment for all staff, clients, residents, and customers as a matter of mission as well as in accordance with the VOSH Emergency Temporary Standard for the COVID-19 Pandemic.
- This employee travel form applies to personal, family, or business travel of any kind.

Employee Travel Registration Form
Employees traveling for personal or approved business reasons:
- It is advised that employees complete the Travel Registry after they return from their trip, but prior to returning to any YWCA facility. Your direct supervisor will advise on next steps as needed. As a threshold matter, we request that employees who are traveling please register on the Employee Travel Registry so the YWCA may determine next steps relative to remote work or potential quarantine, in accordance with CDC guidelines and any recommendations from local health authorities.
